  • Here you will have to provide the following information;
  • Social Security Number
  • Date of Birth
  • Zip Code
  • Once you have provided these information, you can then click on the blue "Continue" button which is below the "Clear" and "Cancel" buttons. Note that all fields (information) is required before you can click on "Continue".
